Also, if you would like the APDT to send a personalized card acknowledging your gift of kindness, please be sure to include the recipient's name and address as well.

Memorials will be posted and acknowledgment cards mailed within two weeks of receipt of donation.

They will remain on the web site for one year and will be featured in the November/December issue of the APDT Chronicle of the Dog.

All submissions are subject to approval.
